Just to add to your frustration maybe, I was talking with a doctor friend of mine more than a year ago about smoking and "smoking related" deaths -- the number given by the U.S. govt and American Cancer Society stands at ~400,000 annually. My doc friend said that regardless of how an individual dies (except accidental), if they smoked regularly at any point in their lives, their death is listed as "smoking related."

So, let's say that Jim goes to Thailand, hooks-up with a bar girl on night and gets hepatitis. Jim smoked when he was in his 20s but quit at 37 y.o.; Jim dies from his hepatitis when he's 54...the corner's report will list Jim's death as hepatitis and smoking related.
